Outlook crashes when ShoreTelConnectUCBAddIn enabled

We just upgraded to the latest build ShoreTel connect and reinstall client connect to the last version. Outlook still crashes when trying to create new meeting in Outlook and ShoreTelConnectUCBAddIn enabled.

When trying to create new meeting in Outlook and ShoreTelConnectUCBAddIn disabled. Outlook working fine.

Any idea why this is happening?
